Requesting Accommodations

Accommodations are determined on an individual basis through an interactive process that includes input from the student, licensed and credentialed evaluators, Office for Accessible Education (OAE), and relevant university officials, when necessary.

To request accommodations, students must follow the process summarized below:

1. Contact OAE via phone, email, mail, or in person.

2. Participate in an initial meeting to discuss eligibility requirements, documentation guidelines, and accommodations that, if implemented, would provide equal access to a university-sponsored course, program, or activity. 

3. Submit documentation that meets the university's guidelines for eligibility determination.

Documentation Guidelines:

The disability documentation must be provided by a qualified professional. The diagnosing or reporting professional may not be a relative of the student. The documentation must be current, provide a clear diagnosis of the disability, and include sufficient information to determine the extent of the disability and what accommodations are appropriate. The University recommends following the best practices for documentation guidelines from the Association on Higher Education and Disability (AHEAD). The seven elements of documentation recommended by AHEAD include the following:

  • The credentials of the evaluator(s)
  • A diagnostic statement identifying the disability
  • A description of the diagnostic methodology used
  • A description of the current functional limitations
  • A description of the expected progression or stability of the disability
  • A description of current and past accommodations, services and/or medications
  • Recommendations for accommodations, adaptive services, assistive services, compensatory strategies, and/or collateral support devices

Note: With the exception of ESA requests, OAE will gladly review various types of documentation to help satisfy guideline criteria.  However, our forms were created to collect the specific information required.

If determined eligible for accommodations, students must:

  • Academic accommodations--Identify the specific courses for which implementation will be necessary and notify all relevant instructors of the need for support via the ClockWork Student Portal.

  • Non-academic accommodations (i.e., housing and dining)--Collaborate with OAE to properly notify relevant staff or department heads of the specific need. 

  • Continue to engage in an interactive process with the OAE and the course instructor or deparment head to discuss specific implementation and ongoing effectiveness of the adjustment.
